FINANCIAL OPTIONS:

INSURANCE COVERAGE:

We do not accept Insurance as a form of payment. Payment in full is due at the time services are rendered. As a courtesy to our patients who have dental insurance, we can file your insurance electronically. While dental insurance is certainly beneficial, as a general rule dental insurance plans usually only pay a small portion of periodontal procedures. We do not file secondary or medical insurance. Our practice will provide you with a treatment plan that explains all costs that may be incurred. All treatment plans are valid for 6 months.

APPOINTMENTS/CANCELLATIONS:

Each individual appointment requires specialized time and preparation to offer you the best possible care available. This time is reserved especially for you. Therefore, we require all of our patients to give a minimum of 48-hours notice in the event that they are unable to keep their scheduled appointment. Patients who fail to keep their appointment, and do not give at least 48 hour notice of a need to cancel the appointment will be charged $50.00 per hour as per our office policy.
Please note that repeatedly canceling appointments or failing to arrive for your appointment at the scheduled time may result in our discharging you from our practice.
